Photodynamic therapy (PDT) is a dynamic interaction between the photosensitizer, oxygen, and light. Its use for treatment of age-related macular degeneration, actinic keratosis, esophageal cancers, and non-small cell lung cancer has been approved by the FDA. LEDs are semiconductor devices in which light is generated as a result of electron-hole recombination. Production of light in an LED follows the same basic working principle of a diode laser, but LEDs do not have a resonant cavity for stimulated emission and light is emitted spontaneously. Hence, the output light from an LED is incoherent with broad spectral width and large beam divergence. Light with any color can be generated by LEDs but with a wider spectral width (approximately 5% of the central wavelength) in comparison with diode lasers. This results in a lower absorption efficiency by the photosensitizer. LEDs generally have a lower output power compared to a diode laser. LED-based PDT light sources have been made in various sizes and arrangements. LEDs have the advantage of being fairly low-cost and portable and can be used for PDT in regions with little or no access to electricity or other advanced medical care. Clinical applications that benefit with using LEDs are with superficial tumors. LED arrays can be used for endoscopic or interstitial application. One of the concerns of using LEDs, especially for interstitial PDT, is the thermal effect resulted from the low electrical to optical conversion efficiency.

• 1.Blue light (wavelength 405nm-420nm) provides a very superficial penetration even with a high-intensity narrowband light source. It is primarily absorbed by the epidermis of the skin. Once it is absorbed, blue light targets P. acnes by stimulating porphyrins which then produce intracellular singlet oxygen and thus inducing bacterial death.

• 2.Red light (wavelength 630nm-640nm) penetrates human tissue superficially, with approximately 80% of the energy being absorbed in the first 2cm. Red light energy has a significant effect on mitochondrial stimulation, which increases the production of ATP and in turn boosts fibroblast activity. This leads to an increase in cellular turnover, superficial circulation, and an anti-inflammatory emission.

• 3.Infrared (IR) light (wavelength 800nm – 900nm) energy penetrates tissue at a greater depth than the blue and red light. Approximately 50% penetrates to 8cm and decreases to less than 1% at 20cm (NASA study). Infrared energy is known to heat tissue and its effects are well documented for therapeutic pain management. For cosmetic applications, IR stimulates the NaK+ pump which increases cell membrane permeability; facilitating equilibrium of cellular pH, while increasing nutritional absorption and elimination of waste byproducts.
